Fill out the form as completely as possible. Please see below options for sending.

Step 2. Send the form to the registrar’s office by fax, as an e-mail attachment, or via USPS. Please note that it's not currently possible to submit a transcript request form via this website.

Step 3. Make a payment online: pay without registration www.gurnick.edu/online-payment/pay or create an account and log in www.gurnick.edu/online-payment. Alternately, a check or money order can be mailed to the registrar’s office with your completed request form. (If paying by check, allow two additional weeks to the transcript processing time.)

Official transcript fee: $15.00 each (please allow 2 weeks for delivery)
Rushed official transcript fee: $30.00 each (available within 5 business days of request receipt)
Unofficial transcript fee: none
