Fill out the form as completely as possible. Please see below options for sending.

Step 2

Send the form to the registrar’s office by fax, as an e-mail attachment, or via USPS. Please note that it’s not currently possible to submit a replacement diploma request form via this website.

Step 3

Make a payment online: pay without registration www.gurnick.edu/online-payment/pay or create an account and log in www.gurnick.edu/online-payment. Alternately, a check or money order can be mailed to the registrar’s office with your completed request form. (If paying by check, allow two additional weeks to the diploma processing time.)

Diploma Replacement fee: $15.00 each (please allow 2 weeks for delivery)
Rushed Diploma Replacement fee: $30.00 each (available within 5 business days of request receipt)

FAQ

Q: How do I fill out the Replacement Diploma Request Form online?

A:  You can NOT fill out the form online. Please print out the form, you will need to sign it at the bottom for it to be valid.

Q: I forgot my username and password or can no longer log in to Gurnick website online payment section. How can I make a payment online?

A:  Pay without registration here: www.gurnick.edu/online-payment/pay.

Q: What if I want to pick up my Diploma on campus?

A:  You can pick up your Diploma at the corporate office in San Mateo with prior arrangements or have your Diploma mailed to your campus.

Q: I need my Diploma as soon as possible! What is the fastest way to get it?

A:  Request a rushed Diploma and pay the associated fee online. If you have a special circumstance and need your Diploma sent before 5 business days, please call the registrar to see if alternate arrangements can be made.

Q: Can I pay for my Diploma by check? How long does that take?

A:  All checks must be received at the corporate office and deposited from there. It can take up to one month to receive clearance for a check, and this can delay the processing of your request. If you have a deadline or time-sensitive reason for requesting your Diploma, we strongly encourage you to pay online or submit a money order with your request form.

Q: I don’t have a printer. What other options do I have?

A:  If you do not have a printer, you may have a copy of the request form sent to your home by calling the registrar’s office, or you may request one in person at your campus (you will need to fill out the request form there and the campus will fax it to the registrar).

Q: How do I prove that I made a payment online?

A:  Proof of payment is not necessary; our accounting department will post your payment and notify the registrar.
